Скрыть эстетические дефекты лица и тела – рубцы, депигментированные участки кожи, частичную алопецию – косметология и пластическая хирургия не всегда в силах.

В этих случаях сегодня все чаще применяют метод косметологической дермопигментации.

За последние 7-10 лет произошел стремительный рывок в области эстетической медицины и косметологии. Красивое, ухоженное тело стало визитной карточкой как современного делового человека, так и домохо- зяек, ведущих активную социальную жизнь. Врожденные нарушения пиг- ментации кожи, такие как витилиго, и приобретенные дефекты – шрамы, рубцы, стрии и т.д., могут значительно снижать самооценку их обладателя с образованием последующих поведенческих комплексов. Некоторые из вышеперечисленных дефектов поддаются коррекции при помощи терапевтической косметологии, другие – исключительно радикальными методами, относящимися к эстетической медицине.

Послеоперационное рубцевание. 

Пластическая хирургия, как уже говорилось выше, поз- воляет исправлять грубые эсте- тические нарушения. Но, к сожа- лению, одно из нежелательных побочных явлений пластической операции – образование постопе- рационных рубцов. Некоторые из них можно исправить при помо- щи косметологических уходов, а другие – только замаскировать. В этом случае на помощь приходит эстетическая и художественная дермография. Чем больше конт- раст между цветом здоровой кожи и цветом рубца, тем выразитель- нее результат. Конечно, необходимо уточнить, что в любом случае результат закрашивания эстетических дефектов татуажем будет выглядеть как тональный крем. Этот участок не будет загорать и при малейшем изменении цвета здоровой кожи будет выделяться. В нашей практике был случай, когда после крупной аварии и последующей пластической операции лицо пациента было покрыть рубцовой тканью на 70%. Методом дермопигментации рубцовая ткань была закрашена под цвет здоровой кожи, что дало потрясающий косметический эффект.

Сюда же стоит отнести депигментированные или рубцовые следы, остающиеся после уда- ления лазером кожных образо- ваний, чаще всего родинок. Эти участки закрашиваются методом дермопигментации под цвет здоро- вой кожи или восстанавливается удаленная родинка.

Слагаемые успеха

Следует заметить, что каждый пациент, обращаясь к специалис- ту по дермопигментации для уст- ранения эстетических дефектов, ожидает получить 100% резуль- тат. К сожалению, работа с кожей, как с живым, «капризным» материалом, полна непредсказуемых моментов. Успех работы зависит от взаимопонимания и доверия между специалистом и пациентом. Пациент должен получить макси- мум информации о предстоящей процедуре, четко представлять каждый этап работы и важность домашнего постпроцедурного ухода. Важно объяснить пациен- ту особенности выбора пигмента, учитывая входящие в них цветовые стабилизаторы.

Со стороны специалиста в данном вопросе возможно небольшое давление на пациента, так как окончательный результат может спрогнозировать только он. Необходимо предупредить пациента о возможной дополнительной коррекции (80% случаев), так как пигмент на участках, где кожа прак- тически заменена соединительной тканью, ложится очень плохо. Во многом успех процедуры зави- сит от домашнего постпроцедурного ухода, выполняемого самим пациентом. Тщательное выпол- нение предписаний специалиста помогает ровному, аккуратному восстановлению тканей и снижает риск осложнений – попадания вто- ричной инфекции и рубцевания.

Cosmetology and plastic surgery not always can help to hide aesthetic face and body defects, such as scars, depigmented skin spots, partial alopecia. In these cases, the method of cosmetic dermopigmentation is used more and more often.

Plastic surgery, as it was told above, allows correcting crude aesthetic defects. But, unfortunately, appearing of postsurgical scars is one of the undesirable side effects of a plastic surgery. It is possible to correct some of them with the help of cosmetic care, but the others can be mased only. In these cases aesthetic and artistic dermographia can help. The more is contrast between healthy skin color and scar color, the more expressive the result is. Of course, it is necessary to mention that the result of filling aesthetic defects with the help of tattoo will look like a toner any way.

This skin area will not get a tan and in case of any change of healthy skin color it will be notable. There was an occurrance in our practice when after a huge crash 70% of the patient's facewas covered with cicatrical tissue. By the method of dermopigmentation the cicatrical tissue was painted like healthy skin. It gave fantastic cosmetic effect.

Here we can rate depigmented or cicatrical spots that appear after deleting skin formations by laser (molesarethemostoften). Thisspots are being filled with the help of dermopigmentation like healthy skin or a deleted mole can be re-established.

SUMMANDS OF SUCCESS

It is necessary to warn the patient about a possible additional correction (80% of cases) as pigment on the areas, where the skin is changed by connective tissue, is passed too bad. Often success depends on the home care after the procedure. Painstaking fulfilment of the specialist's prescriptions leads to smooth and neat tissue renewal and reduces the risk of complications (repeated infection and scarring).

STAGES OF THE PROCEDURE

Pigment matching is the first and very important stage. Specialst estimates healthy skin color and injuried area color. The selected pigment should be a little warmer than the healthy skin color, warm tints should dominate (yellow, orange) as it is important to remember that pigment will loose part of its warm colors after the tattoo healing and skin renewal.

Of course, accuracy of matching depends on the specialist experience. Pigment density defines the density and brightness of the area where it will be plotted. The more the contrast between the healthy skin and the injuried area is, the densier the pigment should be. It is necessary also to take into account the tint of the injuried skin (if it has). You can make a test on the first procedure. The matched pigment is being plotted on the small area and the result is being estimated after the total recovery. After the necessary corrections the specialist can fill the whole injuried area.

After the pigment selection it is necessary to select the method of plotting and prepare the equipment.

There are several main techniques used for dermopigmentation: contouring, filling, stumping. During the procedure specialist can choose one of them or their combinations. Contouring is used to make an accurate form or to plot the lines. It is used for lips, areola and hair imitation tattoo.

Filling makes it possible to fill required skin area homogeneausly. It is used in almost all kinds of the procedures.

Stumping is necessary to create a gradual crossing between the filled and healthy skin and for thin fillng as well.

Soreness is inevitable due to the fact that dermopigmentation process is connected with tissue injury, vessels and effectors. Power of pain sence depends on the individual pain limit, general physical and mental status etc. Methods of local anaesthesia (application and injectable) are used to make the procedure as comfortable as it is possible. For the application anaesthesia the cream "Emla" is being used. The cream is applied on the work area and is covered with the special "Tagoderm" plaster. (The cream "Emla" is interesting because of two anesthetics it contais - lidocaine and prilocaine. It provides maximal skin penetration due to the fact that it turns to the emulsion at a temperature of 25° С and due to the great amount of water it penetrates deeply into the skin). Such a bandage creates the necessary greenhouse effect that partly reduces the skin barrier function.

It is necessary to mention that each patient who comes to dermopigmentation specialist to delete aesthetic defects waits for a 100% result. Unfortunately there are a lot of unpredictable moments while working with skin, as it is live and "capricious" material. The success depends on the mutal understanding and trust between the specialist and patient. Patient should have maximum of the information about the procedure, clear envisage each stage of the procedure and realise the importance of the home care after the procedure. It is importante to explain to the patient specific features ofthe pigment choice taking into account color regulators they contain.

   For producing a natural effect an artist should not change the lip shape considerably, especially if the lips are pretty pronounced, because the pigment is inside the skin and the border of a natural shape will be visible, especially when direct rays fall on it. Such variant is more suitable for a client who wants enhance the lips with the help of permanent make-up. An artist should pay attention to the central part of the upper lip, a so- called Cupid’s bow. A precisely outlined and corresponding to the philtral columns proves the lip youth. But it is possible to restore the lip youth, suffice it to accentuate this area with a fine line using white pigment. 

   By the way, white pigment is useful not only to the lip contour, but also for the bottom of wrinkles, which smoothes them out.

Перманентный макияж бровей.

Существует два направления в косметическом татуаже бровей: «натуральный» - когда бровь смотрится абсолютно естественно, и «декоративный» - имитирующий

декоративную косметику (карандаш, тени). «Натуральный» образ гармонично сочетается с цветом волос, кожи, радужной оболочки глаз. Корректирует форму, максимально захватывая свои волоски. Техника выполнения – «отдельными волосками» или комбинированная с «растушѐвкой». «Декоративный» перманентный макияж даѐт полную волю фантазии. Здесь возможна любая форма и цвет, с единственным правилом: это не должно выглядеть татуировкой, т.е человек находящийся рядом с владельцем пигментированных бровей должен догадываться только об искусном нанесении декоративной косметики. Чаще всего технология нанесения декоративного метода - растушѐвка.

Первое, что должен сделать мастер на процедуре перманентного макияжа, выяснить все пожелания клиента: форму, цвет, методику нанесения. Идеально, если клиент придѐт в салон в ежедневном макияже и любимой одежде, так будет легче понять его видение своего лица и комфортные цветовые сочетания. Необходимо выяснить будет ли дама менять цвет волос и причѐску, в каком направлении. Особам, любящим эксперименты лучше подобрать нейтральные цвета бровей, подходящим под цвет естественных, или повторяющих цвет корней волос.

После подробного обсуждения мастер выполняет эскиз формы косметическим карандашом, или тенями. Надо постараться сымитировать окончательный результат процедуры. Некоторые мастера допускают ошибку, обводя только контур брови или используя не подходящие для эскиза средства (маркер, гелевую ручку), т.к. клиент, глядя на такой рисунок, не сможет понять что ждѐт его через месяц, а некоторые просто не обладают должным воображением и спасти процедуру сможет только авторитет специалиста.

Когда форма и цвет полностью согласованны следует заполнение необходимой формы договора, в которой фиксируются все данные клиента, возможные противопоказания, пигмент и т.д. А так же ставится подпись о получении памятки по уходу.

Следующий этап – анестезия. Учитывая, что дермопигментация сопровождается нарушением кожных покровов, болевые ощущения неизбежны. Смягчить их и сделать процедуру более комфортной помогут местные аппликационные средства. Лучшим из них, по праву, считается крем «Эмла» 5% или мазь «Мезонамб». Крем накладывается на область бровей и накрывается аклюзивной повязкой. Время экспозиции 20 – 40 мин, чем больше, тем глубже проникнет крем и сильнее будет эффект. Снимать анестезию лучше по очереди, та бровь, на которой не работают «проморозится» ещѐ лучше.

Первые движения машинкой фиксируют набросок на коже. Здесь действует закон – бровь не должна быть обведена чѐткой линией. Для красивого результата даже декоративный стиль требует естественности. По этому контур наноситься очень легко отдельными штришками, точечками и т.д., идеально, если специалист сможет выполнить перманентный макияж бровей не обводя бровь контуром – это настоящее мастерство! Для ещѐ большего осветления намѐтки можно разбавить пигмент специальным составом - Mix, который разбавит цвет и сделает его прозрачным. До тех пор, пока бровь не будет полностью прокрашена пигмент с кожи не удаляют, что бы ненароком не повредить рисунок.

После этого следует основная процедура, бровь прокрашивается выбранной техникой: отдельными волосками или растушѐвкой. Если техники комбинируются, то первыми выполняются волоски. Чем тщательнее будет выполнена работа, тем меньше вероятность дополнительной процедуры – коррекции. Не нужно жалеть времени, что бы внимательно разглядеть работу под лампой-лупой, можно просветить кожу специальным точечным фонарико м.

Не мало важный момент «симметрия» - вторую бровь нужно сделать такой же. Помочь в достижении симметричных линий может: а) зеркало – мастер может посмотреть на клиента сзади, немного сверху через зеркало, б) цифровой фотоаппарат с экраном – сфотографируйте результат и оцените его в экране.

Сразу после процедуры брови будут достаточно тѐмными, предупредить клиента об этом желательно заранее, что бы не было неожиданности. Краска сейчас лежит во всех слоях кожи и сверху, образуя корочку. Для достижения аккуратной, тонкой корочки в день процедуры нужно промыть перманентный макияж раствором Миромистина или Хлоргексидина, до полного удаления лимфы, после чего наносить тонкий слой ранозаживляющего крема (Бепантен, Траумиль С). Крем наносят несколько дней, до того момента, пока не сойдѐт тѐмная корочка. Мастер не должен удивляться, если через 5 – 7 дней позвонит клиент и скажет, что у него получился очень светлый цвет, или что у него «ничего не осталось». За то время, пока брови тѐмные глаз привыкает к новому образу и потеря цвета воспринимается как не правильно выполненная процедура. Дело в том, полностью все слои кожи обновятся только через 28 дней, и никак не раньше. В течении этого времени новая форма будет покрыта слоем отшелушивающегося эпидермиса, как молочной плѐнкой. И только после полного восстановления кожи цвет проявится и будет таким, какой сохранится несколько лет.

Первую неделю после процедуры категорически нельзя планировать никаких распаривающих процедур: баня, сауна, бассейн, поход к косметологу. А так же запрещается загорать на открытом солнце и в солярии. Нельзя самостоятельно удалять образовавшуюся корочку, во избежание потери цвета и образования рубца.

Окончательный результат, через 1 месяц, следует показать специалисту и решить, нужна ли повторная процедура. Коррекция считается нормальным явлением, и обвинять мастера в том, что он не «сделал всѐ с одного раза» не стоит. Кожа – живой организм, и она так же участвует в процессе перманентного макияжа.

Качественный перманентный макияж держится на лице 1.5 – 2 года, чем темнее, контрастнее цвет, тем дольше он будет выгорать. Для более продолжительного эффекта стоит защищать брови от прямых солнечных лучей.

There are two directions in cosmetic eyebrow tattooing: the first one is natural, when eyebrows look natural, and the second is decorative, which imitates decorative cosmetics (pencil, shadow). The natural image matches the hair, skin, iris colour. It corrects the shape leaving most hairs. The technique is called “Haitstrokes” or combined with “Shading”. The decorative permanent makeup gives vent to imagination. Any shape or colour is possible, the only rule is that it should not look

like a tattoo, it should seem just a decorative make-up. The most frequent technique for this type of permanent makeup is shading.

Before the procedure, an artist should ask the client about the shape, colour and application method. A client wearing their daily makeup and preferred clothes is best of all, it helps to understand their view on the face and comfort colour combinations. It is necessary to find out whether the woman will change the hair colour and hairdo, in what direction. Ladies who love experiments should set their choice on neutral eyebrow colours matching or repeating the natural colour of the root of the hair.

After a detailed discussion, the artist makes a sketch of the shape with a pencil or shadow imitating the final result of the procedure. Some artists make a mistake outlining the contour with unsuitable means (a marker pen, gel pen), because the client, looking in the mirror, will not be able to imagine the result, and only authority of the specialist might save the procedure.

When the artist and the client agreed on the shape and colour, they should fill in the informed consent blank recording all the client's data: possible contra-indications, pigments, etc. Besides, the client signs for post-procedural care instructions.

The next stage is an anesthetization. Taking account of dermopigmentation skin damage painful sensation is inevitable. Local application means help to ease the pain and make the procedure more comfortable. The best is by right 5% Emla cream and Mesonumb ointment. The cream is applied on the eyebrow area and covered with an occlusive dressing. Exposition time is 20-40 minutes. The longer the deeper gets the cream and will be more efficient. It is better to remove the anesthesia in turn, first from the eyebrow to be treated, while the other remains under an anaesthetic.

First movement with a machine fix the sketch on the skin. The law is the eyebrow should not be outlined in a precise line. For a nice result even the decorative style requires naturalness. That's why the contour is applied easily in separate strokes, points, etc. It is perfect when a specialist can perform a permanent makeup without contouring the eyebrow, this is the unsurpassed skill! For more lightening the sketch,

the artist can dilute the pigment with a special solution Mix, which will dissolve the colour and make it transparent. Unless the eyebrow is completely painted the pigment is not removed in order not to harm the sketch inadvertently.

Then it's time for the main procedure, the eyebrow is painted in a chosen technique: in separate hairs or shading. If the techniques are combined, then hairs are made first. The more thorough the work is performed, the less is the probability of additional procedure, i.e. correction. The artist should spare no time to look at the work carefully under the lamp with a magnifying glass or light the skin with a special pinpoint torch.

Symmetry is of no small importance. The second eyebrow should be made the same. With the purpose one can use: a) a mirror – an artist can look at the client from behind, a little from above through the mirror: b) a digital camera with a screen – make a photo of the result and look at it attentively.

When you perform my new luxury technique  "Babydoll" it stays visible  but not looking artificial.  Color transitions and a light gloss effect, creates a combination of different density and brightness in pigments, which are applied to the prepared lips creating the effect of "Pumped Up" lips.
